It's pretty curious how the pod got inverted, but didn't take the TV with it. I tend to believe that the hitch is unhitched, and it's just chains holding things together. I also think that the tires look OK. I suspect the fender got torn off during the roll-over.

No obvious (to me) reason why this happened, but I am inclined to believe they lost control. There could be any number of reasons why. Was there a news story along with the picture? Do we know where this happened?

Now that I got a good look at the undercarriage, how difficult would it be to add a second axle between the fresh & gray water tanks?

Oh yeah. I see they had some kind of carrier on the rear hitch. It's not entirely clear to me what that is. I do see a fair amount of "turf" attached to the rear stabilizers.

Whether there was sway control is unknown. I copied the image and enlarged it.

If any tire had blown, it would have been the passenger (door) side. That is where the fender is ripped off. The driver's side tire looks intact. However, it would be unclear whether the tire and fender damage is the cause of, or the result of the rollover. Being that it was one with the fresh water tank in the back, it could have been an issue of too much weight in the back if the tank were full. However, that can't be seen in the picture. Whatever the cause, it looks like while the trailer rolled, the TV did not. Thankfully, it looks like people were shaken up, but not injured.
