Has anyone installed a safety bar in their shower? Our shower doesn't have smooth walls so we can't put in a suction model. Are there decent studs to screw into or do I have to use wall anchors? (if so what kind)

The only way I would be confident using a safety bar in any RV would be a to remove the wall panel, install appropriate bracing in the wall, re-install the wall panel and install the safety bar against the bracing. RV walls and the aluminum studs are not up to providing the adequate support needed IMHO.
